The donor, court or other third party who is responsible for the decision regarding the frequency, duration and type of test to be performed will designate the specifics of testing prior to implementation of the individual random program.
Contact details and if/when the donor presents for testing will be documented in the random program file. Results will be reported to the entitled parties. Donors must pay non-refundable contact fees up-front in order to enroll in the program.
Pre-payment discounts may be implemented, if multiple tests are pre-paid at the onset of the program. You may not retrospectively apply the discounts to tests taken at an earlier time.
The discounted rates are as follows for pre-paid testing prior to implementation of an Individual Random Program:
- 4 Tests = 10% Discount
- 8 Tests = 15% Discount
- 12 Tests = 20% Discount
- 16 or more Tests = 25% Discount
